92979    Intravascular ultrasound (coronary vessel or graft) during diagnostic evaluation and/or therapeutic intervention including imaging supervision, interpretation and report; each additional vessel (List separately in addition to code for primary procedure)

Answer:

Code 92978 may be billed once per encounter, as per the CPT descriptor, for the first major coronary artery studied (including its branches).

Code 92979 may be billed up to four times (although very rare) if intravascular ultrasound (IVUS) was performed in the additional four major coronary arteries.
