A physician is asked to insert a central venous tunnelled catheter. They first attempt the RIJ by US. They access the RIJ with a wire and catheter but are unable to pass the wire and catheter beyond the innominate vein. They abandon this access. They are successful with a LIJ approach. Should one code the unsuccessful catheterization of the RIJ?

Since the right internal jugular (RIJ) was punctured, and abandoned, you may report 36000 modifier XU/59 for the catheterization. If a venogram was performed to verify the blockage, you would report 36000 XU/59 / 75860 (jugular) or 36005 XU/59 / 75710 (innominate) as appropriate via documentation.
